When the referral is only the beginning
You may know a senior needs help with meals, personal care, housing, safety, or support at home. The harder question is what happens after they leave your care. Did the service start? Can they manage day to day? Is there another concern no one has seen yet?
Seniors In Distress gives social workers a way to connect seniors with an Advocate who can look at those questions more closely.
Refer a senior when you notice
Meals, transportation, medication access, or personal care failing
The senior’s living situation raises safety concerns
Isolation, caregiver strain, or possible mistreatment needs attention
A discharge plan may be difficult to carry out at home
The senior needs more support than a single service referral can provide
Help with Medicaid and other benefits
As a Community Partner with Florida’s Department of Children and Families Economic Self-Sufficiency program, we helps seniors navigate
the benefits process at no charge.
We can help with:
Applying for Medicaid and food assistance through SNAP
Gathering and submitting requested documents
Understanding notices and requests for information
Completing renewals
Following up on application status
Social workers can connect seniors with us when paperwork, missing records, or uncertainty about the next step prevents them from moving forward. Eligibility and approval are determined by the administering agency.
After you refer a senior
We make contact. An Advocate reaches out to the senior
or their authorized contact to hear their concernsWe visit when appropriate. A free Quality of Life Check helps us understand their daily needs and living situation
We make a plan. We identify priorities, help connect the senior
with available support, and set follow-up stepsWe keep you informed. With the senior’s permission, we share

Share the concern and contact information for the senior or an authorized contact. We will review the referral and work with the senior to determine the next step.
Early Childhood Development (ECD)
At the heart of the program is Early Childhood Development, targeting children between the ages of 0 to 5 years. It focuses on foundational brain development, emotional nurturing, and early learning.
Through community-based ECD centers, children receive age-appropriate stimulation, structured play, balanced meals, and regular health checks. Parents are also trained in responsive caregiving and positive parenting practices.
Education Access
The program ensures that school-age children have access to quality education by providing school fees subsidies, uniforms, learning materials, and mentorship. Special emphasis is placed on girl-child education and children with disabilities. School retention campaigns and after-school tutoring help improve literacy and numeracy, while partnerships with local schools enhance teaching quality and infrastructure.
Nutrition & Health
Nutrition is a cornerstone of the program, with consistent feeding schemes established in ECD centers and primary schools. Malnutrition screenings are conducted regularly, followed by treatment and dietary supplementation where necessary. Health outreach efforts include immunizations, deworming, vitamin A distribution, and access to clean water and sanitation facilities.

When a plan gets stuck
A housing application may be waiting for records. Transportation may be approved but never arrive. A senior may agree to a service without understanding how to get it started.
These loose ends can hold up an entire plan. Seniors In Distress can help clarify the problem, assist with practical tasks, and follow up with the people involved.

Before getting started, the case manager and Seniors In Distress identify the specific tasks that need attention and agree on who will handle each one. This keeps responsibilities clear and helps the senior avoid repeating the same information to several people.

When instructions are difficult to follow
A senior may leave an appointment with clear instructions but struggle to put them into practice. Opening medication bottles, preparing suitable meals, arranging a ride, or remembering questions for the next visit can become difficult.
These concerns may only become clear when someone sees how the senior lives. A free Quality of Life Check helps bring those difficulties
to light.
When to connect a senior
Appointments are missed because getting there is difficult.
Care instructions are confusing or hard to manage.
Limited mobility makes meals, hygiene, or daily routines difficult.
Family or caregiver support is unavailable or inconsistent.
The senior reports managing well, but concerns remain about daily life.
Bring everyday concerns into view
With the senior’s permission, Seniors In Distress can share observations about food access, living conditions, available support, and difficulties managing daily routines. This information can help the healthcare team understand what the senior is facing between visits.
Questions about symptoms, medications, or treatment are directed to the healthcare provider.
