  We will confirm the location, coordinate a convenient date and time,

• Complete the Quality of Life Check
  An Advocate will spend time with the senior, listen to concerns, and
  review their care, protection, support, and daily living conditions

  
  

• Review Our findings
  We will discuss what we discovered, identify areas needing attention,
  and recommend appropriate services or resources

  
  

• Follow Up
  We will check on unresolved concerns and help determine whether additional assistance is needed
